Feel Free To Email The Completed Packet Back To The Nurse When It Is Completed In Its Entirety.

SPORTS REGISTRATION PROCESS:

1. Log into Parent-Portal

2. Under “Forms”, fill out 2023-24 Winter Sports Participation

3. Under “Forms”, fill out Athletics Health History Update (Winter 2023)

This tab will appear when your student is CLEARED

Physical Exams

All physical exams should be completed by your family physician.

Incomplete exams without a stamp will not be accepted

Be sure to double check for a signature and date on the "Cardiac Professional Development Module" line from the PCP.

Physicals must include a vision screening. Do not leave your PCP office without this component.

Sports Physicals

Any high school student wanting to try-out for sports teams must submit an exam completed within the previous 365 days before the tryout.
