Medication Forms & Seizure Action Plan Paperwork

Medication Policy

All medication to be taken during school hours must be left in the Nurse’s Office and administered by the nurse.

Exception: A parent may come to school to administer medication to their own child at any time.

All medications, including over-the-counter items such as Tylenol extra strength and cough syrup, must have our district form signed by both the doctor and the parent.

No medication will be given without these forms.

Many physicians are able to provide the required form via fax directly to your child’s school. Parents must call the doctor’s office to make this arrangement. School fax numbers are located on the nurse’s webpages.

Dropping off Medication:

A parent/guardian or other adult relative over 18 must bring medication to the school nurse in the appropriate container. The medication must be in its original container or pharmacy-labeled

container. The nurse can not accept pills in envelopes or plastic bags.

Picking up Medication:

All medication must be picked up from the Nurse’s Office by the last day of the school year annually.

Any Remaining Medications Will Be Discarded Within 48 Hours--they may not be held over the summer months.
