Incoming 9th Grader/Student Paperwork

The Nurse's Office must have the following paperwork on file for your student:

1. Copy or New Physical Exam from Primary Care Physician's Office (most recent copy if within 1 year)

2. Copy of Immunizations or Copy of Medical Exemption Form/Religious Exemption Letter

3. Asthma Action Plan (if applicable)

4. Epi-Pen Plan (if applicable)
