Epi-Pen Action Plan Paperwork

If your child self-carries an epi-pen or has an epi-pen in the nurses office for an anaphylactic allergy please have their primary care provider fill out this paperwork for their medication. 

This gives emergency personnel a better understanding as to the extent of your students reaction/allergy.
